Expanding access to fertility care requires careful consideration of how benefits are designed, which services they cover, and their full costs to patients and health plans. A new white paper, The Cost of Assisted Reproductive Technology Treatments, reports findings from a retrospective actuarial analysis of commercial insurance claims examining costs associated with in vitro fertilization (IVF) and intrauterine insemination (IUI), including costs that may be overlooked in conventional benefit estimates.

This retrospective analysis of approximately five million commercially insured patients examines whether guideline-recommended infertility evaluations and treatments were completed before in vitro fertilization. The findings identify substantial care gaps and highlight the potential role of restorative reproductive medicine in supporting diagnosis-directed, comprehensive fertility care.

The International Institute for Restorative Reproductive Medicine presents a proposed definition of infertility and indications for fertility evaluation. The statement recognizes infertility as a clinical condition that points to underlying male and/or female pathology and considers factors including fertility-timed intercourse, pregnancy loss, reproductive health conditions, and age-related fertility decline.

A proposed rule – published May 13, 2026 by the Departments of Treasury, Labor, and HHS – would create a new category of “Excepted Fertility Benefits” in the U.S. allowing employers to offer fertility coverage separately from major medical insurance. While much of the public discussion around fertility benefits has focused on IVF, the proposed rule also includes language relevant to diagnosis, mitigation, and treatment of infertility and infertility-related reproductive health conditions.
